What does the percentage mean?


The percentage shown in the AI writing detection indicator and in the AI writing report is the amount of qualifying text within the
submission that Turnitin's AI writing detection model determines was generated by AI.

Our testing has found that there is a higher incidence of false positives when the percentage is less than 20. In order to reduce the
likelihood of misinterpretation, the AI indicator will display an asterisk for percentages less than 20 to call attention to the fact that
the score is less reliable.

However, the final decision on whether any misconduct has occurred rests with the reviewer/instructor. They should use the
percentage as a means to start a formative conversation with their student and/or use it to examine the submitted assignment in
greater detail according to their school's policies.

How does Turnitin's indicator address false positives?


Our model only processes qualifying text in the form of long-form writing. Long-form writing means individual sentences contained in paragraphs that make up a
longer piece of written work, such as an essay, a dissertation, or an article, etc. Qualifying text that has been determined to be AI-generated will be highlighted blue
on the submission text.

Non-qualifying text, such as bullet points, annotated bibliographies, etc., will not be processed and can create disparity between the submission highlights and the
percentage shown.

What does 'qualifying text' mean?


Sometimes false positives (incorrectly flagging human-written text as AI-generated), can include lists without a lot of structural variation, text that literally repeats
itself, or text that has been paraphrased without developing new ideas. If our indicator shows a higher amount of AI writing in such text, we advise you to take that
into consideration when looking at the percentage indicated.

In this paper, we will be comparing the DARP and SOAP methods after assessing a client

to complete a progress note. We will explore the significance of conducting an assessment and

ways it can aid the healthcare professional. We will then discuss the significance of

documentation and its necessity. Lastly, we will complete by explaining the preferred method

between SOAP and DARP charting method. The following is a brief overview of the client's

details respectively; the client is 30 years old man diagnosed with Generalized Anxiety Disorder.

The client experiences excessive anxiety and worry about different aspect of life, and the other

client is a 24-year-old single lady who experiences chronic stress.

Data, Action, Response and plan(DARP), is subjective on the client's behavior and

health. Data represent the information observed and heard during the session(SUTTLE, 2009).

The client is a 24 years old single lady who is experiencing challenge in concentration at work.

She stated that she lacks interest in her favorite hobbies, as well as insomnia.

The action taken by client is counselling whereby assessment was conducted. Clients’

response was evident by symptoms of chronic stress. Her decline in interest and insomnia, shows

a mental breakdown. She, however, express the wiliness to improve her life. The plan section

defines the future of the therapy. The plan is to begin cognitive behavioral strategies in order to

cope with stress, manage her time. Follow-ups are done to monitor her progress.

SOAP (subjective, objective, assessment, plan), is a method of documentation that is easy

to comprehend during a therapy session. Subjectivity entails clients’ view on their experiences

and feelings. Sometimes it includes the client's close people(Lew et al., 2022). The client stated

to have been experiencing excessive worry and anxiety based on life in particular. The 30-year-

old man stated to been overwhelmed by daily activities as well as facing difficulty in

concentration because of the wandering thoughts. He added to be under depression, losing

purpose in life and change of moods consistently. He added that he has lost mood in fun

activities that made him joyful before.

Objective is based on the data that is observable. This entails diagnostic information,

physical exam findings, laboratory information and so on(Lew et al., 2022). During the therapy

session, the client seemed to be tensed and could manage to maintain an eye contact. He was

restless as he fidgeted his hands and shaking his feet. He spoke rapidly when asked a question

and often interrupted himself while expressing himself due to his wandering thoughts.

Furthermore, he seemed to have difficulty breathing well and sighed often. He stated to feel tired

and sleepy even though he has had enough sleep.

The assessment part combines the subjective and objective information(Lew et al., 2022).

In our case, from the objective and subjective information collected, it can be concluded that the

client is experiencing symptoms of Generalized Anxiety Disorder. Worry, anxiety and

depression are the contributing factors to his daily misfunction. The client no longer enjoys a

task that seemed to be fun previously. He is hopeless in life, which explains the signs of

depression.

The plan is initiated to bring the solution(Lew et al., 2022). The plan is collaborated with

additional drugs, testing, other activities like counselling or new hobbies and many others. In our

scenario, the plan was to curb depression and Generalized Anxiety Disorder. The following were

the strategy;

● Cognitive-Behavioural Therapy, which would aid the client to not only identity the

negative thoughts but also challenge them. This taught him to reframe the thoughts to

positive ones, management of stressful situation and skills to coping any situation and

moment he faces in life.

● Relaxation techniques like mindfulness, deep breathing to help him especially when he

feels anxious as well as bring calmness.

Wright et al. (1998) stated, conducting assessment is vital as;

● It empowers patient to take part in managing their health, since it is likely that patient

will abide by recommended plans if they comprehend their health treatment options and

status.

● It provides efficient communication between the patient and professionals as it boost

comprehension of by professional of their patient care in expressing empathy.

● Thirdly, it helps in keeping track of patient's progress in time by comparing previous and

current information in order to attain an accurate report on effectiveness of treatment, and

future plans.

● Help professionals find potential risk and pushing factors in order to develop proactive

measures and reduce the likelihood of reoccurring and future problems.

documentation is necessary and importance because;

● It aids in identification of risk and managing them, which permits organization to keep

track of potential issues, assess their effect and find solutions to curb them.

● It helps in decision-making by assessing previous results, and pushing factors. For

instance, in assessing the objective and subjective data in the SOAP method.

● It provides evaluation of performance and feedback in order to keep track of client's

progress, for instance in both DARP and SOAP method especially in observation of

behavior of client and their response.

● It ensures continuity in planning and resilience since the progress is documented with

strategic plan in order. Client will be able to abide by the plans stated before and progress

forward to treatment plans.

Data, Action, Response and plan(DARP), is subjective on the client's behavior and

health. SOAP (subjective, objective, assessment, plan), is a method of documentation that is easy

to comprehend during a therapy session. Conducting assessment assist healthcare professional in

patient empowerment, effective communication, curb risk and keep track of progress.

Documentation is vital because it helps in decision-making, performance evaluation, continuity

in planning and identification of risk. I prefer the DARP method because, firstly, it does not take

much time. Secondly, it captures all the subjective and objective information under data. Lastly,

through the response, the client can express themselves in order to find the root problem.
