Antecedents Behaviours Consequences


Record Date & Time Describe what was happening Describe what actually happened; what Describe what was the outcome or what
of Observation & immediately prior to the incident; who did the behaviour look like; sound like; happened as a result of the behaviour; what
your initials. was there (or had just left); what were feel like? Write it out like stage directions did the person do; what did other do; how
they or others doing (or about to do)? for a play so someone who has never did the situation change from how it was
seen it could accurately act it out. before the incident?
