Madam:

With reference to Memorandum No. DM.-PFO-2018-0482 signed by Undersecretary


Jesus LR. Mateo on March 27, 2018, the training on the Results-Based Performance
Management System (RPMS) for Teachers and Multi-Factored Assessment Tool (MFAT). A
SLAC session on Multi-Factored Assessment Tool (MFAT) and the Philippine Standards for
Teachers and the Results-Based Performance Management System should be
implemented.

With this regard, Kasiglahan Village Elementary School would like to inform your
good office and submit the SLAC/Training Design for implementation. The SLAC Program
was based from Municipal Roll-Out on the Impelemtation of PPST-RPMS on August 8-20,
2018 held at Kasiglahan National High School. The time that was included in the SLAC
Program is in Compliance of DepEd Order no. 9 s, 2005 entitled, Instituting Measures to
Increase Engage Time-On-Task.

Attached herewith are the LAC / Training design and training matrix for your
references.
