CHECKLIST FOR CHEST PHYSIOTHERAPY

INSTRUCTIONS FOR THE EXAMINER: observe if the participant is performing the correct sequence, score 1 for each point or mark 0 if the task is
not done. Calculate the score and total obtainable score is 15

1.PRELIMINARY ASSESSMENT
 Verify physician's orders and identify patient 0.5
 Explain procedure to the patient. 0.5
 Check patient's pulse and respiratory rate. Auscultate 0.5
chest. Allow 30-45 minutes after patient's completion
of a meal.
 Perform hand hygiene 0.5
2. EQUIPMENTS
 Pillows 1
 Stethescope 1
 Sputum cup 1
 Towel 1
CHEST PERCUSSION
1. Position the patient (appropriate position for lung 1
segment to be drained)

2. Place folded towel or thick pad across patient's chest 1
over area to be percussed (adults and older children).
3. Cup hands with fingers and thumbs closed. Begin 1
percussion over lung segment by flexion and extension
of wrists.
4. Percuss back and forth or in a circular motion, not 1
continuously over one spot. 30-45 seconds to 2-3
minutes per segment depending on amount of
secretions
CHEST VIBRATION
5. Instruct the patient to inhale slowly and deeply. Vibrate 1
the chest wall as the patient exhales slowly
6. Place one hand on top of the other affected over area 1
or place one hand place one and on each side of the rib
cage.

7. Encourage patient to cough frequently 1


8. Auscultate chest. Check patient's pulse and respiratory 2
rate, Document the areas (lobes) percussed.
